You may have a talent for home repairs. You may be able to rebuild your computer. You may even be able to put together a whole room of furniture armed only with flat-box kits and an Allen wrench. But, please—don’t try do-it-yourself orthodontics!

Now that generic clear aligners are available, you might consider giving them a try to save some money. But is straightening your own teeth really a good idea? Before you are tempted, let’s look more closely at the products and the dental science involved.

Invisalign®

• Invisalign clear aligners are used by orthodontists and dentists with experience in custom treatment for your smile. A 3D image of your teeth will be captured by the iTero Element® scanner. Using special software, your doctor can map out each projected shift in your teeth, and even show you a projection of your finished smile!
• Your Invisalign aligners will be tailored to fit your teeth precisely using the 3D scan and 3D printing. They are made from SmartTrack® material, a product specifically engineered for a perfect, comfortable fit. Invisalign aligners are even trimmed to fit your individual gumline to prevent irritation.
• When your first sets of Invisalign aligners arrive at our Halifax office, Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard will check for fit, answer any questions you might have about use and care, and let you know what to look for and what to expect. Your progress will be monitored with visits every six to eight weeks. (And for parents of teens, Invisalign aligners can offer blue “compliance indicators” to let you know they are being worn the 20-22 hours a day necessary for the best and fastest results.)

Generic Aligners

• You might be required to make a putty mold of your own upper and lower teeth, which is not the easiest thing to do well, and to take selfies of your teeth.
• The aligners will be sent to you in the mail. They are generally made of hard plastic with generic gumlines. There will be no one to tell you if the aligners fit properly.
• They are sometimes less expensive because there is no in-person medical supervision. A dental professional working for the company will look at the model created from molds you submit, and recommend a series of aligners to correct the problems he detects by looking at the model and your selfies. This supervisor will not be able to assess the overall dental health of each patient to make sure teeth and gums are healthy and ready to start treatment, and will not be able to tell if the teeth are moving properly or improperly once the aligners are in use.

Finally, while generic aligners may potentially have some success in minor tooth straightening, they are not created to deal with complex bite issues or malocclusions.  In fact, using generic aligners with no supervision can cause more serious dental problems than a patient started with.

Sure, sometimes a do-it-yourself project turns out well. But your teeth and bones are too important for home improvement. You may have noticed that we specialize in orthodontics and dentofacial orthopedics. And while most people we talk to have heard of orthodontics, many are confused by the dentofacial orthopedics part of the title. Today, Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard and our team thought we would explain the difference.

While orthodontics entails the management of tooth movement, dentofacial orthopedics involves the guidance of facial growth and facial development, which occurs for the most part during childhood, and is a reason why kids are often the best candidates for receiving dentofacial orthopedic therapy. Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard will examine and monitor your child’s growth to determine when starting treatment will be most effective. If your child begins orthodontic treatment before his or her adult teeth have erupted, it is known as Phase-One treatment. During this phase, Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard will use treatments designed to correct your child’s jaw growth and make sure that the jaw bone is properly aligned before beginning the next phase of treatment, which usually involves placing braces to straighten your child's teeth.

Dentofacial orthopedics is also used to treat adult patients at Metro Orthodontic Specialists, however, this process may involve surgery. With our younger patients, we know the jaw bones are still forming, making it easier for our team at Metro Orthodontic Specialists to control bone growth and tooth movement. Adults, however, are a different story; their bones are no longer growing, and their jaw bones have hardened, so it is more difficult to adjust the bite and move teeth into proper alignment. Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard may recommend surgery to adjust the jaw bone and establish the proper bite alignment before beginning treatment.

Because our team at Metro Orthodontic Specialists is skilled in both areas, we are able to diagnose any misalignments in the teeth and jaw as well as the facial structure, and can devise a treatment plan that integrates both orthodontic and dentofacial orthopedic treatments.

Now that you’ve graduated from your braces or aligners, you might think you’re finished with orthodontic cleaning tips. Not quite yet! Your retainer needs love, too—not just because it can look or smell “less than appealing” without your help, but because it’s good for your dental health.

Removable Retainers

If you have a Hawley retainer or a clear retainer, cleaning it whenever you remove it is a great idea. Unappetizing white patches mean plaque or mineral deposits from your saliva have dried and hardened on your retainer’s surface. And if you notice an unpleasant taste or odor, it probably means that germs and bacteria have made themselves at home and are growing in and on your appliance.

You don’t want old plaque or new bacteria in your mouth! Here are some ways to keep your retainer clean and fresh:

• After wearing it, you can clean your retainer with a toothbrush, but don’t brush too vigorously. You might scratch it. Use a soft or even extra-soft brush to clean out all the nooks and crannies, and then rinse.
• Use cleaning products which have been recommended by Dr. Paul Bourque, Dr. Kathy Russell, Dr. Brien Stackhouse, and Dr. Magda Barnard if your retainer needs a deeper clean. Denture cleaners, retainer cleaners, and even toothpastes can be too abrasive or cause discoloration, so use the products made for your specific retainer.
• Let your retainer dry before storing it in a dry place after cleaning. Shutting a damp retainer into a closed case provides bacteria with the damp, dark environment they thrive in. (PS—clean your case regularly, too!)
• One of the benefits of a clear retainer is that it’s almost invisible. And you can help keep it that way by removing it whenever you eat or drink. Your retainer can become stained from colored foods or liquids.

Fixed Retainers

A permanent retainer is a small piece of wire that is custom-fit and bonded to the inside of selected teeth to keep them from shifting. Because food particles and plaque can accumulate around a fixed retainer, brushing after meals and at least twice a day is recommended. Bacteria and plaque cause bad breath, cavities, and tartar buildup, and removing tartar might require removing your retainer.

While a fixed retainer can be tricky to clean, there are techniques and products that make cleaning easier and more effective:

• An orthodontic toothbrush with a smaller head might reach behind your teeth more comfortably.
• Don’t forget to floss! Using a floss threader will help you get that wriggly floss behind the retainer and between your teeth.
• Try a water flosser. These handy devices direct a stream of water right into hard-to-reach places for more thorough cleaning.
